GH-releasing hexapeptide, GHS-R1a agonist
About GHRP-2
GHRP-2 (pralmorelin) is a synthetic hexapeptide from the family of growth hormone-releasing peptides, developed from the pioneering work of Cyril Bowers.
It acts on the ghrelin receptor (GHS-R1a) in the hypothalamus and pituitary and is one of the most potent GH secretagogues in its class. In Japan it has been used as a diagnostic agent for GH deficiency in adults.
Quick guide · 5 key points
GHRP-2: a potent GH releaser
GHRP-2 is a synthetic hexapeptide that triggers growth hormone (GH) release through the ghrelin receptor. Here are five things the research shows.
-
1
More GH release than GHRH itself
GHRP-2 is a synthetic agonist of the ghrelin receptor, the same receptor the appetite hormone acts on. In 6 healthy young adults, intravenous GHRP-2 was associated with a GH release greater than that of GHRH, and greater still at the higher dose.1,2

A diagnostic test for GH deficiency
In a Japanese study of 58 patients with severe GH deficiency and 77 healthy subjects, one intravenous dose of GHRP-2 brought peak GH to 84.6 µg/L in healthy subjects vs 1.36 µg/L in patients, with good reproducibility. The test has been shown to tell the two groups apart with high reliability.3

3
More appetite, as with ghrelin
In 7 lean, healthy men, a GHRP-2 infusion was associated with a 35.9% increase in food intake at a free buffet compared with saline. Every participant ate more, similar to what has been reported with ghrelin.2

4
Active by the nasal route, too
In 15 children with short stature, 6 months of intranasal GHRP-2 was associated with growth velocity rising from 3.7 to 6.1 cm/year, a modest but significant increase. It was well tolerated.4

5
Not fully selective
GHRP-2 is not fully specific for GH: in healthy adults it has been associated with rises in prolactin, ACTH and cortisol. Its effect on ACTH and cortisol was similar to that of corticotropin-releasing hormone (CRH).1
